Making a Referral

We regularly work alongside hospital discharge teams and community professionals to support individuals transitioning from acute or inpatient settings into residential care. Referrals to Willowbrook Care Home begin with an initial discussion with our team so that we can understand the individual’s current needs, risks and placement objectives. Once a placement has been agreed, we work closely with families, discharge teams and community professionals to coordinate admission planning and support a smooth transition into the home.
